Maternal and neonatal deaths were considered related to drug-induced periparturient hypocalcemia. Renal impairment has been observed following the administration of zoledronic acid in patients with pre-existing renal compromise or other risk factors [see WARNINGS AND PRECAUTIONS]. In patients with renal impairment, the exposure to concomitant medications that are primarily renally excreted (e.g., digoxin) may increase. Consider monitoring serum creatinine in patients at risk for renal impairment who are taking concomitant medications that are primarily excreted by the kidney. The incidence of all-cause mortality (one in each group) and serious adverse events were similar between the Reclast and active control treatment groups. The percentage of patients experiencing at least one adverse event was comparable between the Reclast and active control groups, with the exception of a higher incidence of post-dose symptoms in the Reclast group that occurred within 3 days after infusion.
- Qualitative, quantitative and micro CT assessments showed bone of normal architecture and quality without mineralization defects.
- Reclassification can occur for various reasons, such as correcting errors in initial classifications, complying with changes in accounting standards, or providing a more accurate representation of the financial position and performance of a business.
- Other adverse fetal effects at this dose included reduced lens, rudimentary cerebellum, reduction or absence of liver lobes, reduction of lung lobes, vessel dilation, cleft palate, and edema.
- Reclassification can also be used to describe moving a note payable from a long-term liability account to a short-term or current liability account when the note’s maturity date is less than one year away.
- The safety of Reclast in men with osteoporosis or osteoporosis secondary to hypogonadism was assessed in a two year randomized, multicenter, double-blind, active controlled group study of 302 men aged 25 to 86 years.
Other drugs may interact with zoledronic acid, including prescription and over-the-counter medicines, vitamins, and herbal products. Tell your doctor about all your current medicines and any medicine you start or stop using. Zometa is another brand of zoledronic acid used to treat high blood levels of calcium caused by cancer (also called hypercalcemia of malignancy). This medicine also treats multiple myeloma (a size bm and momentum effects and the robustness of the fama‐french three‐factor model type of bone marrow cancer) or bone cancer that has spread from elsewhere in the body. When submitting a reclassification request through Job Builder, UCSB’s job description management system, at times it is helpful for the employee to complete a reclassification questionnaire as part of the reclassification documentation. Please consult with your Compensation Analyst so see if a questionnaire should be submitted.
The Process for Reclassifications
Renal function measured prior to dosing and at the end of the 12 month study was comparable in the Reclast and active control groups [see WARNINGS AND PRECAUTIONS]. There were 4 patients (2.6%) on Reclast vs. 2 patients (1.4%) on active control with local site reactions. Reclast solution for infusion must not be allowed to come in contact with any calcium or other divalent cation-containing solutions, and should be administered as a single intravenous solution through a separate vented infusion line. Reclast Injection is available as a sterile solution in bottles for intravenous infusion. One bottle with 100 mL solution contains 5.330 mg of zoledronic acid monohydrate, equivalent to 5 mg zoledronic acid on an anhydrous basis. Reclast may interact with diuretics (water pills), antibiotics, other medicines that can harm your kidneys, cancer medicines, or nonsteroidal anti-inflammatory drugs (NSAIDs).
Lifestyle changes that help promote healthy bones include increasing weight-bearing exercise, stopping smoking, limiting alcohol, and eating well-balanced meals that contain adequate calcium and vitamin D. Since you may also need to take calcium and vitamin D supplements and make lifestyle changes, consult your doctor for specific advice. There is another zoledronic acid product that is used to treat bone problems that may occur with cancer. Ask about reliable forms of birth control while using this medication and after the last dose. If you become pregnant, talk to your doctor right away about the risks and benefits of this medication. Institutional leaders have told us that the current framework, which applies a single label of highest degree offered, is too limiting and does not adequately describe the full scope of work taking place across their campuses.
- In such cases, the staff member should voice his/her disagreement with the evaluation to his/her manager, administrator or HR consultant.
- In patients naïve to previous treatment, a greater therapeutic response was also observed with Reclast (98%) relative to risedronate (86%).
- Tell your dentist that you are using this medication before you have any dental work done.
- Once an employee has been performing the full scope of higher level duties for 30 days or more, the department may submit a request for review.
- Renal impairment has been observed following the administration of zoledronic acid in patients with pre-existing renal compromise or other risk factors [see WARNINGS AND PRECAUTIONS].
However, there were no patients with adjudicated serious adverse events of atrial fibrillation in the Reclast treatment group. The safety and effectiveness of Reclast for the treatment of osteoporosis is based on clinical data of three years duration. All patients on bisphosphonate therapy should have the need for continued therapy re-evaluated on a periodic basis.
Drugs & Supplements
Acute renal failure requiring hospitalization and/or dialysis or with a fatal outcome have been rarely reported. Transient rise in serum creatinine can be correctable with intravenous fluids. Reclast was administered once as a single 5 mg dose in 100 mL solution infused over at least 15 minutes. Risedronate was given as an oral daily dose of 30 mg for 2 months. The safety of Reclast in men with osteoporosis or osteoporosis secondary to hypogonadism was assessed in a two year randomized, multicenter, double-blind, active controlled group study of 302 men aged 25 to 86 years. Reclast is also used to treat or prevent osteoporosis in people who will be taking certain steroid medicines for 12 months or longer.
What happens if I miss a dose?
An interaction between two medications does not always mean that you must stop taking one of the medications; however, sometimes it does. Speak to your doctor about how drug interactions should be managed. Before using zoledronic acid, tell your doctor or pharmacist if you are allergic to it; or to other bisphosphonates such as alendronate or risedronate; or if you have trouble breathing (wheezing) after taking aspirin; or if you have any other allergies. This product may contain inactive ingredients, which can cause allergic reactions or other problems. Once the Reclassification Committee has reviewed the position, the HR consultant will report back to the department administrator/director regarding the decision and any recommended and approved changes.

Caution is advised when bisphosphonates, including zoledronic acid, are administered with aminoglycosides, since these agents may have an additive effect to lower serum calcium level for prolonged periods. This effect has not been reported in zoledronic acid clinical trials. In the osteoporosis trials, local reactions at the infusion site such as itching, redness and/or pain have been reported in 0% to 0.7% of patients following the administration of Reclast and 0% to 0.5% of patients following administration of placebo. Drug interactions may change how your medications work or increase your risk for serious side effects. Keep a list of all the products you use (including prescription/nonprescription drugs and herbal products) and share it with your doctor and pharmacist.
Employees covered by collective bargaining agreements must refer to the agreement to determine appropriate salary changes. Overall, the compensation analyst works closely with managers and supervisors to assist them in determining the most appropriate salary increase per individual. Future Projects — We only classify duties being currently performed, not those that might be added to a job down the line. As such, we suggest that reclassification request be delayed until new projects or assignments have begun.
Links to the various questionnaires can be found, below, as well as in the “Forms” section of the HR website. The Supervisor would download the questionnaire, give to the employee being considered for reclassification to complete and later upload to the “Attachments” section of the “Action Justification” tab of the online request. To initiate the formal reclassification process, the Department Head or Business Manager submits an online request by going into Job Builder, completing a reclass action, and submitting it to Compensation for review. The online request must include all of the required pieces of information (see below) or it will be returned to the department with a request for additional information.
Using a global view of HMS, HLC and the University, the committee ensures that each position is reviewed with consistent criteria and is fairly evaluated based on those criteria alone. Director/Administrator – The role of the administrator or director is to provide guidance to the staff member and supervisor on the reclassification process and the completion of the necessary paperwork. The administrator/director also prepares materials that are included with the request. These include an organizational chart, old job descriptions, a comparison of the job with others in the department/unit, and provides a context for understanding the job within the larger organizational structure. Reclassifications should be documented and supported by appropriate explanations to ensure transparency and facilitate the understanding of the changes made. This documentation is crucial for auditors, management, and stakeholders who rely on financial statements for decision-making purposes.
Institutions that are in the R1 or R2 designations are not included. Since 2005, the methodology for classifying R1 institutions has been based on a complicated, 10-metric formula that uses normative and relative scores and places a cap on the number of institutions that can be classified as R1. The result is an opaque process and a moving target that makes it impossible to determine exactly what an institution must do to become classified as R1. This has created unintended competition between institutions that are left to guess what it takes to receive the R1 designation.
Despite the evolution of many institutions of higher education and the ways in which Americans pursue postsecondary credentials, the classification system has not changed considerably since its first release in 1973. Another example of reclassification arises when a company stops using one of its buildings and puts the building up for sale. In that situation, the journal entry description might be, “To reclassify the X building from property, plant and equipment to long-term investments.” In accounting, the term reclassification is often used to describe moving an amount from one general ledger account to another. Supervisor – The supervisor’s primary role is to work with the staff member on the development of the job description and other necessary information included in the HMS Human Resources Reclassification Request Form.
Do not start, stop, or change the dosage of any medicines without your doctor’s approval. Your doctor may direct you to take calcium and vitamin D each day. If you have Paget’s disease, it is especially important that you take the directed amount of calcium and vitamin D during the 2 weeks after your zoledronic acid dose. Vitamin D and calcium are very important to prevent low levels of calcium in the blood. Tell your doctor right away if you have any symptoms of low calcium such as numbness/tingling (especially around the lips/mouth) and muscle spasms.
